NIU OneCard Request
(Do Not Use if You Live in DeKalb or Sycamore)

DO NOT USE THIS FORM IF YOU HAVE LOST YOUR NIU OneCard OR YOUR NIU OneCard IS DAMAGED! You will need to come to the NIU OneCard Office in the Holmes Student Center next to the TCF Bank to replace your card. Their hours are Monday - Friday; 8:30 a.m. to 4:00 p.m.

Forms will be mailed to the NIU remote campus of your choice. You will need to show a photo id to pick up the card.

Forms submitted after 4:30 p.m. on Friday and throughout the weekend (Saturday and Sunday) will be processed the first working day following that weekend. Delivery time to the remote campus will be 10 working days from the date received Monday thru Friday or if submitted on a weekend 10 days from the first working day following that weekend.
